The notice of privacy practices is required by the Privacy Regulations created as a result of Health Insurance Portability and Accountability Act of 1996 (HIPAA). This notice describes how health information about you or your legal dependent (as a patient of this practice) may be used and disclosed, and how you can access to your individually PROTECTED HEALTH INFORMATION (PHI) or identifiable health information.
Please Review This Notice Carefully
1. Our commitment to your privacy:
Healthstar Physicians is dedicated to maintaining the privacy of your individually identifiable health information (PHI).
